Family Practice

5 providers- 20 exam rooms

Patient population : Mixed

8 or 10 hr shifts

M, T, Thurs, F : 7-5 : 30, M-F : 8-430

Weekend : possibility, depends on needs

5 providers (1 day off during week for 4, 1 works 5 days per week) Typically sees anywhere from 15-30 patients per day per provider. Allow for same day scheduling. Attempt to have 2 : 1 ratio for phone calls, inbasket messages, in office patient tasks, nurse visits. Phlebotomy, medication / immunization injections, EKG, ear flushes

Scrubs : Steel Grey

Charge nurse : manager

Our clinic offers onsite pharmacy, behavioral health x 2 days / week, diabetic education and dietician x 2 days per week. We also house a population health coach and community link advocate almost daily to provide resources for our patients.Onsite Xray capable.

Common Diagnosis : Well child appointments to annual physicals, diabetic exams, hypertension, normal aches and pains-coughs and colds, UTIs, Medicare physicals, Covid testing, other point of care testing.

EMR : Epic

Float : Yes, to other CHI Health Clinics as needed

Candidate qualifications : Looking for a dependable, team player who does not require much direction. Willingness to work to the full extent of their licensure based on the needs of the clinics. Someone who is compassionate and kind and strong. Someone able to multitask and has great communication skills.
