The Practice Adelanto Community Health Center ‐ Adelanto, California Ontario Health Center ‐ Ontario, California San Bernardino Health Center ‐ San Bernardino, California Hesperia Health Center ‐ Hesperia, California Hesperia Health Center is a Federally Qualified Health Center. Referral resources include WIC, Dental, Psych, Nutritionist, and MediCal case worker. Facility hours are Mon‐Fri 8am‐5pm. Good mix of patients with peds on staff. 15‐18 patients per day per provider. Point of Care testing with minimal labs (mostly STD).
