PT Home Health Shift : 5X8 M F 8am - 4pm Daily Caseload per tech : 5-6 units Years of experience REQ : 1 First-timers accepted : (Y / N) Possibly after interview CERTS : BLS BJC policy : approx every 6th weekend and 2 holidays with NO Call Is on-call REQ? (Y / N) What is the req? No Will clinician float within scope meet facility needs? No Floating Req to Sister Facilities : (Y / N) : No Open to accommodating block schedule? NA Locals accepted : (Y / N) Yes Pending State License accepted : (Y / N) No RTO Restrictions : No more than 7 days; provide requests and BJC will determine if able to approve or choose another day if possible. Guaranteed Hours : 3 shifts per 13 weeks. The cancelled shifts must be the regular shift length to which the Contractor Personnel is assigned Special Requirements : Department - BJC Home Health in Glen Carbon, Il., consists of a Multidiscipline Team, Geriatrics and middle age, no pediatric patients. Patient Dx - Ortho, cardiac, neuro, pulmonary, Diabetes, other. Case management skills, independent, time manage and organizational skill, patient centered goals, supervising PTA, Team Player, knowledge, professionalism, communication skills, flexibility, positive attitude, provide excellent quality care.
