About The Position

Job Title : Certified Occupational Therapy Assistant

Shift : Monday to Friday, Days

Hours per Shift : Guaranteed 32 hours

Experience : Minimum years of experience preferred, New Graduates are welcome

License : Current state license required

Certifications : COTA certification required

Must-Have :

• Completed prescreen form attached upon submission
• Resume attached
• License verification attached
• Three references provided
• County criminal background check (7 years for all counties lived and worked)
• Federal criminal background check (7 years or more)
• Multi-state criminal search (7 years or more)
• Nationwide sex offender search (7 years or more)
• HHS / OIG search
• GSA search
• Social Security number trace
• One-step PPD or Quantiferon or proof of positive PPD and chest X-ray
• SAM search
• Massachusetts background check (CORI)
• 8 hours of Dementia training

Description :

COVID test required 72 hours prior to start date with results issued 48 hours prior to start date.

SNF experience preferred.
