Job descriptionGraduate of a state-approved school of professional registered nursing. BSN preferred. Current, unrestricted RN license in the state(s) of practice. Minimum of two (2) years of nursing experience, including one (1) year in home care or a closely related field. One (1) year of supervisory and / or case management experience preferred. Current CPR certification. Providing direct care and case management for assigned clients. Collaborating with the regional director and team to enhance services. Planning, implementing, and evaluating care provided. Coordinating client care conferences as needed. Serving as a local on-site clinical resource. Ensuring adherence to agency policies and nursing delegation guidelines. Participating in performance improvement initiatives. Assisting with marketing activities as requested. We provide compassionate in-home care to former Nuclear Weapons Workers who are facing chronic and terminal illnesses due to their previous work environments. As an RN-Case Manager, you will conduct in-home nursing visits for these clients, ensuring they receive the care and support they need.
Your Role :
In this rewarding position, you will :
- Conduct weekly nursing visits lasting 1-2 hours.
- Perform physical assessments, including vital checks.
- Ensure home safety with medical equipment and supplies.
- Fill weekly medication planners and oversee caregivers.
