About the Role :
Tivolisworld LLC is seeking a compassionate and organized Case Manager to support residents in our group homes, veteran housing, and rehabilitation programs. This role focuses on helping individuals achieve personal goals, access essential services, and build pathways toward stability and independence.
Key Responsibilities :
- Develop and manage individualized care plans for residents based on their unique needs and goals.
- Conduct regular assessments to monitor progress and adjust plans accordingly.
- Connect residents with healthcare providers, mental health services, employment programs, and other community resources.
- Maintain regular communication with residents to provide emotional support and encouragement.
- Coordinate care with internal staff and external service providers to ensure continuity and quality of services.
- Maintain accurate and confidential records, documentation, and reports.
- Advocate on behalf of residents to ensure access to appropriate services and support.
- Support residents in crisis situations, including referrals to emergency services when needed.
- Participate in team meetings and contribute to continuous improvement of resident care strategies.
Requirements
Bachelor's degree in Social Work, Psychology, Human Services, or a related field (required).2+ years of case management or direct service experience, preferably in housing or behavioral health settings.Knowledge of local community resources, public assistance programs, and housing support systems.Strong organizational, communication, and interpersonal skills.Ability to work independently, maintain boundaries, and handle sensitive situations with professionalism.Valid driver's license and ability to travel locally as needed.Benefits
