Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

An opportunity is available for an experienced Home Health Registered Nurse (RN) with strong clinical skills to provide exceptional nursing care in patient homes. This contract position involves delivering patient-centered and high-quality nursing services to homebound individuals, helping them manage their health through assessments, education, and ongoing support.

Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and performing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. The RN will closely monitor patient progress, communicate changes to the interdisciplinary care team and physicians, and maintain accurate documentation using Home Care Home Base (HCHB). Collaboration with physical therapists, occupational therapists, social workers, and home health aides is essential to ensure seamless, coordinated care. Participation in case conferences and adherence to regulatory standards are key aspects of this role.

Qualifications / Desired Experience :

• Active RN license in the state of practice, in good standing
• Minimum one year recent home health nursing experience with field work preferred
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage flexible scheduling
• BSN preferred
• Familiarity with Medicare and home health compliance standards
• Experience managing a caseload of 2532 points weekly is a plus

Schedule :

Full-time, Monday through Friday, 8 : 00 AM 5 : 00 PM

Approximately 35 patient visits daily with daily travel to patient homes within the assigned territory (mileage reimbursement provided)
