Job descriptionRegistered Professional Nurse providing safe, competent quality care based on nursing theory and research to ICU patients. Delivers patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational standards. Assesses and evaluates patient care needs using critical thinking in patient care management. Integrates assessment and intervention skills in nursing care delivery. Collaborates with interdisciplinary healthcare team in plan of care development, implementation, and evaluation. Prioritizes patient care including teaching, rounding, coaching, discharge planning, and delegation. Educates patients and caregivers about plan of care, transition, health promotion, and disease prevention. Communicates effectively with patients, family, and healthcare team members. Manages assignments within Modified Primary Nursing Care Delivery Model and allocates resources effectively. Role models Mount Sinai Relationship Centered Care through various patient care actions such as Admission Welcome, Bedside shift Report, HELP protocols, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD / RN Unit Collaboration. Considers patient values, preferences, cultural diversity, and expressed needs in care. Establishes therapeutic relationships with patients and families. Uses evidence-based practices to understand patient perceptions of care. Initiates service recovery and escalates concerns appropriately. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff use of evidence-based practices to understand patient perceptions. Conducts Purposeful Hourly Rounding addressing the 4 Ps. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in ICU patient population. Implements process improvement strategies based on institutional and evidence-based procedures. Engages in peer and institutional review processes. Collaborates on quality improvement projects, evidence-based practice activities, and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements high reliability principles to avoid high risk behaviors and ensure patient safety. Escalates potential safety hazards or gaps following institutional chain of command. Utilizes institutional IT for nursing documentation supporting quality and performance improvement initiatives. Registered Professional Nurse (RN) licensure required. BLS (Basic Life Support) certification required. Unit : Intensive Care Unit (ICU)
