- Registered Nurse (RN) – Home Health / Assisted Living Job Type : FULL TIME / PERMANENT Location : West Hartford, CT 06110 Schedule : Monday–Friday | 8-hour shifts | 40 guaranteed hours per week Pay rate : $35 / hr - $38 / hr About the Role We are seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality patient care in home health and assisted living settings . The RN will be responsible for patient assessments, developing personalized care plans, supervising support staff, and ensuring the highest standards of nursing practice in collaboration with the care team. Key Responsibilities Conduct comprehensive admissions and assessments to develop individualized care plans. Monitor and evaluate patients’ conditions, updating care plans as needed. Provide education and guidance to patients, families, and caregivers regarding disease management and self-care. Supervise Home Health Aides (HHA) and Licensed Practical Nurses (LPNs) to ensure quality and continuity of care. Coordinate with interdisciplinary team members, including OT, PT, MSW, and speech therapists, to optimize patient outcomes. Document care provided and report changes in patient condition to physicians and other healthcare personnel.
