About The Position

Summary : The Social Worker provides support, counseling, and advocacy services to individuals and families facing social, emotional, or health-related challenges. They assess client needs, develop care plans, and connect clients with community resources.

Responsibilities :

• Assess clients’ social, emotional, and environmental needs.
• Provide counseling, crisis intervention, and support services.
• Develop and implement individualized care or treatment plans.
• Coordinate resources and referrals to community or healthcare services.
• Maintain accurate documentation and uphold confidentiality standards.

Requirements :

2 - Year experience Required,

Epic experience Required,

BLS Certification,

MSW (Masters of Social Work).

Preferred :

LCSW.
