Summary : Provide stabilization case management, support services and employment support to formerly homeless clients residing in Middlesex County other area housing. Liaison with area shelters, SMOC staff, supportive housing staff and local landlords. Case management includes initial and ongoing assessment with regular follow up and referrals / linkages to community-based services.

Primary Responsibilities :

Knowledge and Skill Requirements :

Organizational Relationship : Directly reports to Program Manager.

Physical Requirement : Must be able to drive and travel across Middlesex County and surrounding areas via personal vehicle. Must be able to transport clients in personal vehicle and accompany to appointments / interviews. Must be able to sit or stand for prolonged periods of time. Must be able to operate a computer and complete extensive paperwork. Must be able to ascend and descend multiple flights of stairs. Must be able to lift and carry up to 30lbs.

Working Conditions : Travel via car is required. As part of the responsibilities of this position, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.

Remote Work Option : Remote work is permissible in some positions at SMOC depending on the key functions and responsibilities. The Case Manager position is eligible to work from home 0-20% of the week in scheduling coordination with the department manager.
