Description :
The Physical Therapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives.
They will develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability.
The Physical Therapist will work with the individual to prevent the loss of mobility.
Essential functions and responsibilities include performing patient evaluations by obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data.
The Physical Therapist will establish an individualized plan of care and select treatment techniques that address specific impairments identified in the evaluation and in accordance with physician orders.
Ongoing reassessment of the patient and their response to treatment will be conducted.
Initiation and management of the discharge plan will begin at the first visit and continue throughout treatment.
The Physical Therapist will supervise and delegate to ancillary staff and students consistently with state laws and department policy.
They will complete legible, accurate, and clear documentation within the established timeframe identified in department policy and procedure and in accordance with accreditation requirements and insurance regulations.
Communication with the patient, family, caregivers, significant others, and members of the healthcare team will promote the maximum benefit of care utilizing AIDET.
Assessment of educational needs of the patient, family, caregiver, or significant other will be conducted, providing education to meet those needs.
Participation in professional growth and development through educational programs, including in-service meetings, workshops, and reading current literature concerning the scope of practice is expected.
Active participation in quality improvement activities will be required.
Providing an environment conducive to safety for patients, visitors, and staff is essential.
Compliance with appropriate and approved safety standards is required.
Functioning within the scope of practice as defined by the state's practice act is mandatory.
Other related duties as required and directed will also be performed.
