Your Specific Responsibilities :

This is advanced-level work in a Community Health Improvement program, promoting, maintaining, and improving individual and / or community health by assisting individuals and communities to adopt healthy behaviors. Complex tasks are performed under limited supervision where general policies may not have been previously established and standard methods may not be available. The job requires an extensive knowledge of community dynamics and an ability to be an effective agent for change. This position will serve as a resource to assist individuals, other professionals, and the community to make policy and systems changes associated with substance use prevention. This position specifically serves as the lead for the Health Liaison and Overdose Data to Action grants.

Collaborates with health specialists and civic groups to ascertain community health needs, determine availability of services, and to develop goals using a written work plan. Promotes health discussions in schools, industry, community agencies and faith-based organizations to encourage policy or systems changes related to substance use prevention though the Okaloosa County Community Health Improvement Plan (CHIP) goals, strategies, and objectives. Mobilizes the community to define and address prevention and systems change and develops partnerships to address CHIP goals.

Promotes the Community Health Improvement initiatives among community partners, businesses, childcares, faith-based organizations and individuals. Provides direct support to community partners to ensure successful implementation of health promotion strategies. Communicates extensively with persons outside the organization. Tailors communication to the needs of the audience. Represents the organization to the public, government, and other external sources. Provides information to supervisors and fellow workers. Information can be exchanged face-to-face, in writing, or via telephone / electronic transfer. Develops and maintains cooperation between public, civic, professional, and voluntary agencies.

Conducts activities to increase awareness of the impact of substance use on individuals, families, groups, and communities. Prepares and disseminates educational and informational materials that are linguistically and culturally appropriate. Develops constructive and cooperative working relationships with others. Provides consultation, education and expert advice on Health Liaison and Overdose Data to Action program components and other community health initiatives, including technical, systems-related, or process related topics. Translates or explains information in a way that members of the community can more easily understand issues related to health outcomes, substance use, access to care, and other community health initiatives. Assists community members to establish systems for implementing community health initiatives, such as 5210. Implements plans for Healthy Okaloosa sectors, and Okaloosa County Health Improvement Partnership (OCHIP) groups.

Collects and analyzes data to identify individual and / or community needs prior to planning, implementing, monitoring, and evaluating programs or services. Uses information and data to recognize differences or similarities and sense changes in circumstances or events. Modifies behavior or approach based on this information. Develops multiple approaches for implementing plans in a culturally competent and appropriate manner.

Submits reports on a regular basis per grant requirements for Health Liaison and Overdose Data to Action grants. Collaborates with colleagues and / or community partners to ensure correct and timely reporting on all indicators. Completes purchases for programmatic activities / initiatives / projects.

This position supports the community health assessment (CHA) and CHIP activities related to programmatic overlap and community health priorities. This position may serve as a coordinator of internal or external workgroups or assist in developing and implementing public health campaigns. Other responsibilities may include developing educational materials, writing minutes and agendas, completing reports, assisting with survey or focus group administration, and writing grant applications. Attends training and maintains competencies. Remains knowledgeable of procedures and technical skills necessary to obtain demographic and community health data on Okaloosa County residents. Investigates best practices to inform, educate, and empower the community regarding public health initiatives. Prioritizes work and uses time wisely. Maintains confidentiality of all information within the health department.

Performs all other duties as assigned in support of the Agency’s mission and vision.

Required Knowledge, Skills, and Abilities :

Skilled in operating a variety of audiovisual and computer equipment.

Ability to plan and coordinate the development and implementation of Health promotion and public health education programs and projects.

Ability to collect and analyze data.

Ability to prepare a wide variety of written material such as reports, memoranda, pamphlets, curricula, lesson plans and grant requests.

Ability to establish and maintain effective working relationships with others.

Ability to facilitate groups and engage the community in collaborative partnerships.

Skill in effective communication in written, oral, and electronic formats.

Ability to identify and analyze problems and develop plans or procedures to correct these problems.

Ability to use logic and analysis to identify the strengths and weaknesses of different approaches.

Ability to determine the long-term outcomes of a change in operations.

Skill in compiling, organizing and analyzing data for the purpose of monitoring and evaluating

Knowledge of principles and processes for providing needs assessments, quality service, alternative delivery systems, and customer satisfaction evaluation.

Qualifications :

Bachelor’s degree or higher in Health Education, Health Promotion, Public Health or other health-related field preferred.

CHES, CPH, or MCHES preferred.

Valid U.S. Driver's License required.

A minimum of 6 months of experience in the past 5 years working and / or volunteering in health education, health promotion, and / or leading community health improvement activities.
