Clinical Requirements :

We are looking for a home care RN case manager who is comfortable with thefollowing :

  • Provide in-home assessments for skilled and unskilled clients
  • Provide thoughtful care to patients and families based on a personalized Plan of Care
  • Monitor and respond to changes in the patients conditions and needs
  • Supervise nursing assistants and caregivers
  • Provide care coordination with other BrightStar Care and outside professional care teams
  • Proactively update patients families on plan of care health changes and concerns
  • Work well in with a care team environment as well as independently on their own in the field
  • Time and priority management skills balancing field and office duties
  • Leadership and mentoring skills

    • Requirements :

  • Current unrestricted RN license in Utah
  • Certified CPR and negative TB skin test or chest x-ray
  • Minimum one (1) year of documented experience required in a home care agency primary care clinic or health facility preferred
  • Licensed driver with valid automobile insurance
  • Shared on-call responsibilities
  • Travel to homes within Salt Lake Davis and Summit Counties
  • Average workload of 25-30 hours per week
