Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Provide expert nursing care in patients homes as a Home Health Registered Nurse (RN) in the Indianapolis area. This contract opportunity offers a full-time, 13-week assignment focused on delivering compassionate, patient-centered care to diverse homebound populations. With a schedule Monday through Friday from 8 : 00 AM to 5 : 00 PM, you will manage a caseload averaging 2532 points weekly, typically comprising 35 patient visits per day.

Key Responsibilities :

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and document patient progress using the Home Care Home Base (HCHB) electronic system.
• Educate patients and families on treatment plans, medications, and preventive care.
• Collaborate closely with interdisciplinary teams including therapists and social workers.
• Ensure compliance with agency and regulatory standards, participating in case conferences and continuing education.
