Purpose :
- Provide case management services for the clients of The Salvation Army Harbor Light
Summary :
Facilitate didactic groups and case management servicesPrimary Duties and Responsibilities :
Conducts didactics / educational lecturesMaintain case records in accordance with Harbor Light standardsServe as Case Manager to treatment clientsAssists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)Participates in the review of all clinical issues, activities and program policies and proceduresAttends all clinical staff meetings, and team conferencesParticipates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skillsQualifications :
Ability to determine needs and to work with multi-problem individualsAbility to develop rapport, trust and confidence in client relationshipsMust possess good communication and teamwork skillsStrong organizational skillsTB Test and police clearance requiredAuthorization to work within The United StatesEducation / Experience :
A bachelor's degree in social work or an equivalent comparable human services field is requiredExperience in case management preferredExperience with mental health and / or substance abuse populations preferredComputer Skills :
Excellent computer skills requiredExperience using electronic medical records systems preferredReasoning Ability :
