Description :
The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care based on nursing theory and research.
Delivers patient care by incorporating the principles of Relationship Centered Care and through the application of the nursing process.
Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care.
Educates patients and caregivers about their plan of care, promotion of health, and prevention of disease.
Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team.
Manages assignments within the Care Delivery Model of Modified Primary Nursing.
Role models the tenets of Relationship Centered Care through facilitation of various patient care actions.
Considers the patient's values, preferences, and expressed needs in all aspects of care.
Establishes and maintains a therapeutic relationship with the patient and family.
Initiates service recovery for patients and family members who have concerns.
Demonstrates caring, respect, compassion, empathy, and active listening.
Conducts Purposeful Hourly Rounding on assigned patients addressing the four Ps.
Practices safe and efficient patient-centered care.
Demonstrates clinical skills and knowledge crucial to quality and safety.
Implements process improvement strategies based on evidence-based practices.
Collaborates with interdisciplinary teams for quality improvement projects and research studies.
Contributes to quality and safety practices and required compliance measures.
Implements principles of high reliability to provide a safe environment for patients.
Utilizes institutional information technology by documenting nursing practice.
Performs charge nurse duties as assigned.
Coordinates to ensure supplies are effectively utilized and inventory is sufficient.
Document nursing care accurately and thoroughly in compliance with standards.
Facilitates patient throughput on a shift basis.
Contributes to the environment of care to support clinical colleagues and patients.
Advances clinical competence in nursing practice.
Projects a professional image and communicates with respect and empathy.
Incorporates ethical principles into decision-making for patients and families.
Encourages lifelong learning and innovation for self and others.
Participates in shared decision-making through specialty practice councils and unit initiatives.
