It's more than a career, it's a calling WI-SSM Health at Home Reedsburg Worker Type : PRN Job Highlights : We are looking for a compassionate Home Health LPN for our Reedsburg location. Nurse's cover territory of Sauk, Baraboo, Wisconsin Dells, Adams / Friendship, Lyndon Station, Mauston, New Lisbon, Richland Center, Muscada, Lavalle, Loganville, Reedsburg. Whole territory will only be covered when on call. Areas will be assigned. 5 regular visits in a 8 hour day. Schedule : M-F 8-4 : 30. Every 4th weekend, 2 Holidays / year, on call every 2 weeks or so. PRN requirements : 2 shifts / month. 1 weekend and 1 Holiday Job Summary : Provides care to patients under the direction of a registered nurse or physician, functioning within the scope of license. Job Responsibilities and Requirements : Primary Responsibilities Delivers care to assigned clients and performs direct care in accordance with an established plan of care. Delivers safe care at all times that is in line with physician orders, home health policy and established nursing care standards. Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation. Reports patient-related observations to supervising RN on a daily basis. Administers medications and observes patients for adverse reactions to medications or treatments. Provides documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements and ensures reimbursement. Works in a constant state of alertness and safe manner. May perform point of care testing according to policies and procedures. Performs other duties as assigned. EDUCATION Graduate of an accredited school of nursing or educational equivalency for licensing Experience One year licensed practical nurse experience Physical Requirements Constant use of speech to share information through oral communication. Constant standing and walking. Frequent lifting / carrying and pushing / pulling objects weighing 0-25 lbs. Frequent sitting, reaching and keyboard use / data entry. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of smell to detect / recognize odors. Frequent use of hearing to receive oral communication, distinguish body sounds and / or hear alarms, malfunctioning machinery, etc. Occasional lifting / carrying and pushing / pulling objects weighing 25-50 lbs. Occasional lifting / moving of patients. Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot / leg and hand / arm movements. Occasional driving. Rare crawling and running.
