Licensed Case Manager
Must live in California
We are seeking a full-time Licensed case manager based in California, to work remotely while providing in-person support for patients as needed. This role focuses Advocate for clients with advice and information about treatment options and clinical status, evaluating medical necessity and appropriating resources. We specialize in medically complex patients and are looking for compassionate professionals to provide clinical case management.
Compensation & Benefits
- Salary : $60,000 - $100,000 annually
- Paid Time Off : 3 weeks (2 weeks + 6-7 federal holidays)

Responsibilities
Manage case assignments, develop service plans, track progress, and execute case closuresAdvocate for clients with advice and information about treatment options and clinical status, evaluating medical necessity and appropriating resourcesManage the cost of care wisely, and provide timely and accurate information to payorsTrack and record case information, complete forms, and produce reportsFacilitate care and treatment via scheduling, information sharing, and case coordinationAssess and act accordingly in response to treatment barriers and psychosocial issuesRequired skills and qualifications
Bachelor’s degree in nursing or healthcare-related disciplineThree or more years of experience in case management or nursingExperience with psychological aspects of patient careExcellent interpersonal, verbal, and written communication skillsSuperb critical-thinking, organizational, and analytical skillsAbility to work with all age groups and populationsPreferred skills and qualifications
Certified case manager (CCM) and / or accredited case manager (ACM) certificationProficiency in more than one language
