LPN

A qualified Licensed Practical / Vocational Nurse administers skilled nursing care to patients in their place of residence. This is performed in accordance with providers orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies and procedures.

Responsibilities :

• Assist in the development, implementation, evaluation and revision of plans of care
• Implement the nursing plan of care for each assigned patient
• Submit accurate patient documentation per Agency policy
• Provide nursing services, treatments and diagnostic and preventive procedures as assigned
• Observe signs and symptoms and reports to the providers and RN reactions to treatments and medications, and changes in the patient's physical or emotional condition
• Teach and counsel the patient and family / significant others regarding the nursing care needs and other related problems of the patient at home
• Initiate preventive and rehabilitative nursing procedures as appropriate
• Maintain accurate and complete records of observations, treatments and care of patient
• Complete in-services as assigned. Attend staff meetings / conferences as scheduled
• Perform treatments as ordered by the provider and in accordance with the plan of care
• Observe aseptic technique as required
• Provide extended hours of patient care
• Ensure safe, quality, cost-efficient skilled nursing services
• Perform other job duties as assigned
