Weekend RN Home Health

Location : Monroeville, Pennsylvania, United States

Position Summary :

The Weekend Home Health RN provides patient-centered nursing care within a home-based setting, delivering and managing individualized care plans across Western Pennsylvania and surrounding areas. This position requires strong clinical judgment, reliable documentation skills, and the ability to work independently while traveling between patient homes. Weekend on-call availability is required, along with effective time management to meet charting and care delivery expectations.

Key Responsibilities :

Lead and collaborate in the management and delivery of individualized patient care plans

Provide home health services throughout Western PA and surrounding communities

Spend approximately 20% of the day driving to and from patient locations

Meet all required charting and documentation deadlines

Self-manage scheduling, daily workflow, and patient visit priorities

Schedule and Shift Details :

Friday, Saturday, Sunday from 5 : 00 PM 10 : 00 PM

Scheduled to complete 8 units per day on Saturday and 8 units per day on Sunday

Required to work 2 holidays per year that fall on a weekend

Must complete orientation during two weekdays plus Saturday and Sunday

Travel :

Local travel required within the service area

Required Qualifications & Skills :

Associate degree or higher in Nursing

Active Registered Nurse (RN) license in Pennsylvania

Minimum 1 year of experience in clinical documentation, clinical informatics, or clinical quality

Experience with EMR systems, including Practice Partner or other EMRs

Valid certifications : ARC ALS and AHA ACLS

Ability to meet travel requirements and weekend coverage expectations

Strong time management, documentation, and communication skills

Preferred Qualifications :

RN license number provided at time of application
