• FTE Status : part time or full time opportunities available
• Work Schedule : Monday through Friday 8 : 00-4 : 30 / Every 4th-6th weekend / On Call 1x per week. We serve patients in 9 counties in Northeast Wisconsin.

Job Description :

Join our exceptional home health team! We prioritize work-life balance, allowing you to work independently while being part of a dedicated care team.

As a nurse, you'll utilize advanced clinical skills to deliver skilled, short-term acute care services, focusing on one patient at a time to build trusted relationships. Your role will involve traveling to patients' homes to conduct assessments and provide hands-on care, including the administration of IV medications, wound care, Foley care, and ostomy care.

You'll also provide patient education and coaching to make a meaningful impact on patient outcomes. Collaborate with a multi-disciplinary healthcare team and community resources to ensure the best possible results for our patients.
