Required Experience & Skills

Home Health / Skilled Nursing : Wound care, Wound vacs, Catheter care (Foley, suprapubic, male / female), Ostomy care, Admission of Patient, Communication of patient status changes to RN / MD.

Prefer 1+ year of home health experience.

Charting System Required : HomeCare HomeBase (HCHB) / OASIS (Required).

Note : Proficiency in HCHB preferred. OASIS experience required for staff completing SOC (Start of Care) Home Health ONLY.
