Position Summary
Responsible for the coordination of case management services for children in care under the direction of the Lead Case Manager. This includes coordinating timely referrals to external care providers and consistent communication with relevant stakeholders on referral status, ensuring consistent care coordination communication between medical and behavioral health staff internally and externally, coordinating timely referrals to child advocate services as needed, ensuring that the appropriate health interpretation and translation services are available, and ensuring the coordination of the appropriate post-release services that include the continuity of health-care services.

- Acts as a primary point of contact and liaison for the child, family, identified sponsors and external agencies.
- Responsible for the completion of admission requirements, assessments and Individual Service Plans within regulatory timeframes and including participation of the child, family and potential sponsors.
- Coordinates the timely collection and completion of required documentation for children in care.
- Ensures that the children receive the full range of services identified in the ISP in an integrated and comprehensive manner.
- Works in collaboration with the clinical team and identified stakeholders to provide an assessment of the relationship between the child and the sponsor, utilizing safe screening methods to ensure that the identified sponsor is an appropriate placement for the child.
- Completes within required timeframes, ensuring documentation adheres to agency and federal regulations (i.e. UAC Assessment,Individual Service Plan,Case Reviews, Documentation in internal and external systems
- Makes recommendations for safely release, transfer, and / or initiating home studies
- Provides daily, weekly and monthly submission of required updates to internal and external team members.
- Facilitate regular case management meetings for individuals, groups and families.
- Coordinate with the Everstand staff members and community resources to address the childs service needs.
- In collaboration with the clinical team, evaluates the child and / or family s progress toward goal attainment on a designated basis in line with individual service planning and discharge plan goals.
- Responsible for taking the lead role in making transfer and release recommendations, as well as coordinating the release of children from care in accordance with regulatory timeframes. This includes ensuring timely communication and follow up with requests from external stakeholders.
- Participates in the delivery of case management services 7 days a week, with availability between 8 : 00 a.m. and 10 : 00 p.m. as assigned by the Lead Case Manager to ensure full coverage of the program.
- Facilitate access to and coordination of medical, dental, behavioral health, educational, recreational, legal, religious, and acculturation services in accordance with ORR requirements, ensuring services are documented in accordance with regulatory standards.
- Coordinate access to legal services including Know Your Rights presentations, attorney meetings, and consular visits.
- Facilitate sponsor (re)unification, ensuring safe and timely release planning occurs in adherence to federal timeframes and all safe and timely release efforts are documented fully in the case file. Assist in the coordination of access to interpretation and translation services beyond bilingual Spanish / English capacities, including indigenous and other language needs.
- Demonstrate knowledge and understanding of the grievance system and protection of confidentiality for all children.
- Support mandatory reporting requirements and comply with all related state and federal regulations, including but not limited to Child Level Events, Significant Incident Reporting and PSA (Prevention of Sexual Abuse) compliance.
- Ensure caseload ratios are maintained in compliance with ORR mandates and program capacity limits, meeting face to face (in person) with assigned children at minimum once per week to provide case updates.
- Participate in internal Quality Improvement (QI) processes and ensure audit readiness at all times.
- Maintain effective working relationships with families, children, colleagues, consultants and community agencies.
- Ensure assigned case records meet documentation standards as defined by both federal and state regulations.
- Ability to respond effectively to a fast-paced work environment, to include but not limited to, comprehension and implementation of frequent policy updates, procedure changes and data requests.
- Demonstrates knowledge of and familiarity with both Everstand and Office of Refugee Resettlement Policy and Procedures relating to the care and supervision of Unaccompanied Children. This includes knowledge of legal background and authority governing the practice as related to working with Unaccompanied Children.
- Obtain and maintain certification as a user in Everstands electronic health record software system, Cx360.

Work Environment
This job is on-site and operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Being at a residential and school living environment there may be times you experience conditions in which challenging behavior can be exhibited from our youth to include but not limited to, physical agitation, physical violence and / or emotional outburst.

Position Type / Expected Hours of Work
This is a full-time position. This position regularly requires flexible work hours based on the needs of the program to include evening and weekend work. Eligible for flex schedule. Non-essential for inclement weather.
Travel
Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.
Required Education and Experience
Bachelors degree in behavioral sciences, human services, or social services fields.Fluent in both English and Spanish.Preferred Education and Experience
