Job Summary

Beaver Healthcare and Rehabilitation Center, a premier long-term care and rehabilitation facility, is seeking a highly skilled and compassionate Director of Social Services to join our team. As a leader in senior care, we pride ourselves on providing a unique and individualized approach to our patients and residents. The Director of Social Services will play a critical role in ensuring that our residents receive exceptional care and support, fostering a sense of community and promoting a positive quality of life.

Responsibilities :

• Oversee the social services department, including social workers, activities coordinators, and volunteers, to ensure efficient and effective delivery of services
• Develop, implement, and evaluate programs to meet the social, emotional, and psychological needs of our patients and residents
• Collaborate with the interdisciplinary team to identify and address resident concerns and needs, providing support and resources as needed
• Conduct assessments, develop care plans, and provide counseling to residents and their families
• Recruit, train, and supervise social workers and activities coordinators to ensure high-quality services
• Develop and maintain relationships with community resources, organizations, and healthcare providers to enhance resident services and outcomes
• Participate in quality improvement initiatives and contribute to the development of policies and procedures
• Ensure compliance with regulatory requirements, including those related to social services and patient rights
