Healthy Aging / Complex Care Program

FULL TIME - EXEMPT

Thrive Behavioral Health, the Certified Community Behavioral Health Clinic for Kent County, has been delivering mental health and substance use services since 1976. With more than a dozen locations across Rhode Island and more than 300 employees, Thrive is continuing to grow and meet the needs of the community.

General Summary : Provide assessment evaluations, outpatient short-term psychotherapy and linkage with community service agencies. This role is ideal for a licensed behavioral health professional with a strong background in geriatric care, end-of-life decision-making, and family systems support. The clinician will work closely with older adults and their families to navigate complex medical, emotional, and psychosocial needs, providing person-centered care that honors dignity, autonomy, and cultural values.

Schedule : Mon-Fri, First Shift

Location : Warwick, RI

Salary : $70,000-$84,000 annually, depending on experience.

  • This is a grant funded position through September 2026.

Essential Responsibilities :

  • Provide initial assessment and intake evaluation in client's home.
  • Provide individual, couples, family or group counseling within the scope of one's competencies.
  • Provide crisis intervention.
  • Develop and implement recovery plans and document services consistent with Thrive standards and / or insurance requirements.
  • Collaborate with other providers, agencies, insurance companies and individuals in the client's network of care.
  • Provide relapse prevention, discharge planning, follow-up and aftercare.
  • Provide education, training, and modeling in basic daily living skills, social or parenting skills.
  • Teach clients effective techniques for managing their medication as prescribed.
  • Coordinate community resources, transportation for client, and referrals to appropriate resources including services within Thrive.
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives.
  • Produce, complete, and maintain billing forms, logs, data collection forms, clinical documentation and client records in accordance with Thrive standards and state and federal regulations.
  • Ensure clients have appropriate medical coverage at all time.
  • Accompany clients to community appointment, provides advocacy as needed.
  • Schedule and maintain client hours sufficient to achieve individual / program billing targets
  • Carry and enliven a positive Thrive image consistent with the Thrive's Guiding Principles and professional code of ethics.
  • Qualifications

    Education, Experience and Competencies :

  • Master's Degree in the human service field of social work or counseling required.
  • Minimum (2) year experience working with clients with disabilities preferred.
  • Experience or training in related state and federal assistance programs also preferred.
  • Basic computer literacy skills are necessary.
  • Certifications, Licenses Requirements :

  • Licensed at an independent level (LMHC, LMFT, LICSW) preferred. LCSW's and Associate-level Clinicians will also be considered.
  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards law , or the ability to obtain necessary transportation in order to perform the job responsibilities / tasks is necessary for community-based positions.
