Travel Physical Therapist (PT) – Long Term Care (LTC)

Location : Buffalo, MO

Start Date : 11 / 3 / 2025 | Duration : 13 Weeks

Shift : Days | 8-hour shifts | Monday–Friday | 35 hours guaranteed

Start Window : Flexible between 6 : 00 AM – 10 : 00 AM (must be consistent and communicated with DOR)

Weekends : None | On Call : None

Position Overview

HumanEdge is seeking a dedicated Travel Physical Therapist (PT) to join a Long Term Care (LTC) team in Buffalo, Missouri. The PT will evaluate, treat, and rehabilitate patients with physical impairments to restore mobility, function, and independence using evidence-based therapy practices.

Responsibilities

Conduct patient evaluations and screenings to establish individualized plans of care

Provide therapeutic treatments including exercises, ambulation training, manual therapy, modalities, and wound care

Use standardized assessments and evidence-based interventions

Adjust treatment plans and goals based on patient progress

Collaborate with interdisciplinary healthcare teams

Educate patients, caregivers, and families on therapy goals and safety

Document treatment and progress in EMR per regulatory guidelines

Participate in team meetings and care conferences

Supervise PTAs and rehab aides as appropriate

Adhere to APTA standards, state practice guidelines, and facility policies

Requirements

Required :

Active Missouri PT license

1 year of PT experience

Long Term Care (LTC) or Rehab experience

Strong clinical evaluation and documentation skills

Acceptable :

New grads with Level II clinical rotation experience (not observation-only)
