This role offers the unique opportunity to provide personalized, one-on-one care in the comfort of patients’ homes while enjoying the autonomy and balance that home health provides.

Key Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing disorders in home-based patients
• Develop individualized treatment plans based on physician orders and clinical assessments
• Provide direct speech therapy services to help patients regain or improve communication and swallowing abilities
• Educate patients and family members on exercises and strategies for ongoing progress
• Document patient progress, update care plans, and coordinate with interdisciplinary team members
• Participate in care conferences, in-service training, and performance improvement activities
• Maintain compliance with HIPAA and company policies for patient safety and confidentiality
