Responsibilities and Duties
Deliver basic medical services to adult patients aged 19 to 70sManage a patient panel primarily at nursing home level acuity, including individuals with dementia, distal muscular dystrophy, and traumatic brain injuryProvide care in a facility licensed as an LTACH but functioning as a standard long-term care or skilled nursing facilityCollaborate with interdisciplinary teams to ensure optimal patient outcomesMaintain accurate and timely documentation of patient careAdhere to all facility protocols and care standardsAdditional Information
