FT Community Support Specialist

Description

Join our team at Resources for Independence Central Valley and play a vital role in empowering individuals with disabilities to transition successfully from institutional settings or homelessness to independent living within the community. The Community Support Specialist is under the direct supervision of the CalAIM IL Coordinator. The CS Specialist will work within the CalAIM Program to coordinate transition and diversion services for consumers referred to the program. We are looking to hire for our Fresno Main office. As a CalAIM Community Support Specialist, you will :

• Develop and cultivate strong partnerships with Landlords, Housing complexes, hospitals, rehabilitation centers, and other healthcare facilities to facilitate smooth transitions.
• Collaborate with landlords and discharge planning staff to ensure individuals have the necessary services and supports in place prior to their transition.
• Conduct comprehensive care plan assessments and eligibility screenings.
• Provide direct support and advocacy to individuals throughout the transition process.
• Coordinate and deliver essential goods and items to program participants.
• Maintain accurate and up-to-date case files and documentation.
• Enter data into the required Management Information Systems.
• Participate in regular case staffing meetings.
