The Behavioral Health Registered Nurse (BH RN) is responsible for performing Behavioral Health nursing services, including medical and behavioral health promotion, patient and family education, health maintenance, disease management, and coordination of care as part of an interdisciplinary team. The BH RN assists psychiatric providers by performing clinically effective and compassionate patient care efficiently and effectively.
Essential Duties and Responsibilities : The following description is a general representation of the essential duties and responsibilities of the position.
- Obtain baseline and ongoing data, including health histories, vital signs, and lab tests and result to ongoing health needs and plan of care. Provide nursing assessment and diagnosis, following treatment plans, and community and patient education. Evaluate patient knowledge regarding health, illness, and self-care practices. Promote positive lifestyle and health habits to ensure optimal outcome.
- Perform telephone triage : assess patient problems, advise to assure optimal outcomes, including referral of non-psychiatric health issues to other departments as appropriate. Assist with patient crisis calls, walk-ins, and help facilitate warm handoffs to internal or external providers based on patient need. Provide ongoing education to patients related to specific health and / or psychosocial concerns under established guidelines and with approved educational materials. Remain up to date on telephone care practices.
- Ensure timely and accurate documentation of patient care using Electronic Medical Record, including review of health maintenance records, documentation of patient care, order entry, medication refill requests, appointment scheduling, and in-basket folder and message processing.
- Assist with medication changes, refill requests, coordinating with pharmacies, obtaining prior authorizations, and other prescription orders from the psychiatric providers. Educate patients on side effects and partner with psychiatric providers to manage outcomes.
- Collaborate with providers and staff to identify and resolve issues. Coordinate with Social Workers regarding non-clinical patient needs. Provide mental health education and consultation for Medical, Behavioral Health, and other providers and staff as needed.

Education, Licensure, Experience :
Associate's degree or higher in nursing required.Current, unrestricted license as Registered Nurse in State of Ohio required.Board certification as Psychiatric Mental Health Nurse (PMH-BC) preferred.Current and unexpired BLS certification.2+ years psychiatric or similar nursing experience strongly preferred.Knowledge and Skills :
Computer proficiency in Microsoft Office Suite, EPIC EMR / EHR, database management.Strong written and oral communication skills, including the use of electronic communication devices. Able to communicate effectively and with appropriate level of cultural sensitivity.Demonstrated understanding of community health, behavioral health, medical management and interest in teaching patients, families and groups.Ability to speak a relevant second language is preferred.Essential Physical Demands and Working Environment :
Office work environment, but able to be flexible with changing patient and work environment needs.Evening or occasional weekend work.Ability to walk, sit, and / or stand for extended periods of time. Occasional lifting of up to 25 pounds.Willingness to travel locally or between sites. Active Driver's License and Insurance required.
