Seeking a dedicated Registered Nurse to serve as MDS Coordinator at a skilled nursing facility. The ideal candidate will have experience in long-term care, with knowledge of MDS assessments, Medicare, and care planning processes. This role requires strong attention to detail, clinical expertise, and the ability to collaborate with interdisciplinary teams.

Setting : Skilled Nursing Facility Patient Population : Skilled Nursing / Long-Term Care residents FTE : 1.0 (40 hours / week) Shift : 8 : 00 AM – 4 : 30 PM, Monday–Friday Call : Clinical on-call every 3rd week; weekend rotation approximately every 12 weeks Pay Range : Starting at $34.50 / hr Sign-On Bonus : $15,000 Relocation Bonus : None Licenses Required : RN Experience Preferred : Minimum 1 year RN in LTC with MDS experience
