Position Summary :

The LPN Intake Coordinator is responsible for facilitating the intake, review, and processing of precertification requests for medical services and procedures. This role involves gathering required documentation and ensuring compliance with organizational and regulatory guidelines. The coordinator serves as a liaison between patients, providers, and insurance companies to ensure timely approvals and authorizations.

This position requires a credentialed Licensed Practical Nurse (LPN) with an active license. The LPN Intake Coordinator also supports limited clinical functions, including follow-up on diagnostic testing and medical record review, under the supervision of the Nurse Case Manager.

Essential Duties and Responsibilities :

  • Precertification Intake and Coordination :

Serve as the initial point of contact for precertification requests from providers, patients, and facilities.

  • Collect and document all required information, including patient demographics, and requested services / procedures.
  • Documentation and Compliance :
  • Maintain detailed and accurate records of pre-certification requests in electronic systems.

  • Review submitted clinical information to ensure completeness and accuracy for authorization processing.
  • Adhere to HIPAA and other confidentiality regulations when handling patient information.
  • Identify and escalate issues, delays, or incomplete documentation to management or clinical review teams.
  • Communication and Customer Service :
  • Act as a liaison between patients, providers, insurance companies, and internal teams to communicate the status of precertification requests.

  • Provide updates to providers and patients regarding approval, denial, or pending status.
  • Respond to inquiries regarding precertification requirements, processes, and timelines.
  • Clinical Responsibilities (LPN Scope) :
  • Follow up on diagnostic testing for patients, obtaining medical records and related documentation as needed.

  • Review medical records to determine whether patients meet criteria for case management referral.
  • Bill for activities related to obtaining, reviewing, and documenting medical records and case creation.
  • Collaborate with Nurse Case Managers (RNs) to transition patients requiring full case management services.
  • Maintain accurate and timely documentation of all clinical follow-up activities within the case management system.
  • Support the care coordination process by communicating findings or updates to supervising clinical staff
  • Qualifications

    Minimum Qualifications :

  • Active Licensed Practical Nurse (LPN) credential required.
  • Strong communication and organizational skills.
  • Ability to work independently and manage multiple priorities.
  • Knowledge of medical terminology preferred.
  • Experience with case management or clinical documentation systems is a plus.
