    • Summary :
    • Our mission at Hospice of Anchorage is to help individuals and families prepare for and live well with terminal diagnosis, dying, and grief. We are looking for a full-time Clinical Program Manager who is an innovative and proactive leader capable of taking the clinical arm of HOA into the future, by growing our community outreach programs and augmenting services available to our Anchorage community.

    • RESPONSIBILITIES :
    • _Case Management, Hospice Education and Leadership of HOA Inter-Disciplinary Team : _

    • Leads and supervises the interdisciplinary team which provides clinical care to all In-Home clients and ensures that care plans, approved by the client / family are developed, revised and implemented in a timely manner.
    • Completes initial comprehensive bio-psycho-social assessments that include physical, emotional, financial, cognitive, and social needs of the client.
    • Completes an individualized plan of care in collaboration with other clinical staff members, the client and their families, that includes home safety and fall prevention measures.
    • Provides assistance to clients in gaining access to needed medical, social, educational, and other programs and services as needed.
    • _Bereavement Program Management : _

    • Develops, implements and tracks HOA’s bereavement program in accordance with State of Alaska Volunteer Hospice regulations.
    • Provides 13 months of bereavement support via phone and mail for deceased client’s families.
    • Provides grief support for community members.
    • Organizes and conducts community grief support events, as needed.
    • Leads educational presentations for staff, volunteers, professionals, and community groups on grief and end of life topics.
    • Conducts bereavement evaluations in accordance with Senior In-Home grant requirements.
    • _Outreach : _

    • In conjunction with the Executive Director, leads HOA outreach community events and presentations; develops and maintains good working relationships with a variety of partners, individuals, community agencies / organizations, and governmental agencies in order to promote interagency cooperation and to facilitate appropriate referrals.
    • _Reporting : _

    • Serves as Primary Care Manager and Clinical Manager for the Senior In-Home Grant : Completes assessments, maintains accurate records, completes and submits timely reports and narratives, and follows all rules and regulations of that program.
    • Provides the Board with periodic reports, via the ED, regarding the services provided and recommendations of changes to, or additions of, program services.
    • _Compliance : _

    • In conjunction with the ED, ensures compliance by HOA to all local, state, and federal laws and standards including but not limited to : Department of Labor, OSHA, Alaska State law and regulations, hospice care standards and non-profit accounting standards.
    • In conjunction with the ED, monitors and complies with the Board approved budget and cash flow realities.
    • In conjunction with the ED, reviews all policies, procedures, and programs to ensure they are current and meet applicable standards, laws, and regulations.
    • In conjunction with the ED, provides the Board with information necessary to facilitate Board decisions, including the needs of the community and HOA.
    • Implements a quality control program that ensures effective and efficient delivery of all clinical programs; corrects identified problems and revises policy or procedure as needed.
    • Ensures compliance with all clinical GEMS reporting requirements.
    • _Miscellaneous : _

    • Attends / participates / leads in staff meetings and other meetings as assigned and appropriate to include but not limited to : in-service programs, outreach events, development meetings and fundraising meetings.
    • Informs Resource Center Program Manager of community needs in order for HOA to best supply our library and loan closet.
    • Supports HOA fundraising and development efforts.
    • Performs other duties as assigned.
    • QUALIFICATIONS :
    • Bachelor’s degree in social work, psychology, sociology, human resources or a closely related field with a background in gerontology, long-term care, behavioral health, medical social services, care coordination or other direct social services preferred.
    • Master’s degree in social work mandatory.
    • LMSW or LCSW preferred.
    • Minimum two full-years’ paid work experience providing direct social services preferred.
    • At least two years of paid experience as a supervisor of one or more staff (working in a human services field or setting) in a position with responsibility for planning, development, management or operation of programs and service delivery, fiscal management, needs assessment, program evaluation and similar tasks preferred.
    • Working knowledge of hospice and / or palliative care; knowledge of, and commitment to, the mission of Hospice of Anchorage and willingness to function as part of Hospice of Anchorage’s interdisciplinary team.
    • Working knowledge of laws, rules, regulations and precedents governing volunteer hospice service and activities : the Individuals with Disabilities Education Act (IDEA), The Health Insurance Privacy and Portability Act (HIPPA) and the Americans with Disabilities Act (ADA) including terminology used.
    • Knowledge of community resources for seniors, disability, chronic and acute medical care, housing, long-term care, and other local and national supportive services / resources including entitlement programs.
    • Knowledge of medical, behavioral, habilitative and rehabilitative conditions and requirements of the HSB Waiver Services program’s population.
    • Ability to conduct comprehensive assessment of treatment needs for individuals experiencing physical, cognitive and / or intellectual disabilities.
    • Must be able to pass background check and fingerprinting.
    • Must maintain and provide proof of CPR and First Aid Training.
    • Must provide own transportation to / from work including but not limited to patients’ residences, health fairs, and off-site meetings. Must provide proof of current Alaska driver’s license and good driving record and a vehicle capable of safely driving in climate and conditions within the Anchorage Municipality. Must provide proof of current automobile liability insurance of 50,000 / $100,000 / $25,000.
    • Must provide a current resume listing positions and employers, specifying month and years of experience in positions and the responsibilities and accomplishment of each position pertinent to, and demonstrating development of the distinguishing characteristics of this job.
    • Must provide three references written within the past five years.
    • Must have current negative Tb test within the first two-weeks of employment with HOA and provide proof to the organization.

    • A positive approach to life and death : compassion, patience, flexibility, willingness to help.
    • Working Conditions :
    • Communication with clients and families takes place both on the phone and throughout the Municipality of Anchorage in their homes, whether private residence, assisted living facility, nursing home, or hospital. Must be able to use stairs.
    • Possible exposure to fumes, dust, blood and other bodily fluids, chemicals / cleaners, smoke, and household pets.
    • Requires good health – involves moderate lifting, standing, sitting, and walking.
