Daily Responsibilities For RN Case Manager

• Manage high-risk cases supporting children, adults, and seniors with complex medical, behavioral, and social needs
• Develop and implement individualized care plans such as conducting assessments, care coordination, and engaging members using evidence-based approaches
• Conduct field based member visits

Pay Details : $91,000 - $121,000 (Dependent on years of experience)

Shift : Monday – Friday | 8 : 00am – 5 : 00pm
