  • Starting Pay : $17.50 and up based on experience

    Position Summary : As an Assistant Manager, this position is responsible for setting a good example for co-workers, maintaining good attendance, demonstrating responsibility and possessing a positive attitude. Responsible for assisting the program coordinator to successfully manage the oversight of all community program functions. Assist with both client and staff scheduling and participate in the On-Call rotation. Responsible for implementing and following all Person Centered Plans as well as assisting to train community caregiver staff on how to properly follow Person Centered Plans.

    With the support and direction of the Community Programs Manager, this position is responsible for helping people with disabilities connect to their communities through small group activities such as community events / activities and volunteer opportunities. Provide coaching, training, instruction and supervision for participants to help them become active, productive members of their communities. Assist participants to achieve greater levels of personal, social and vocation independence. Be an integral part of the team in the implementation and documentation of goals and objectives per each individuals person-centered plan.

    Essential Functions and Responsibilities :

    Assists with activities of daily living such as bathing, dressing and personal hygiene.

    Assists with activities involving food purchases, preparation, and portion determination along with following special diets.

    Assists clients with transfers including in and out of bed, automobile, or wheelchair, to the bathroom, and up and down stairs. Some transfers may require the use of adaptive equipment such as a Hoyer Life or EZ Stand.

    Support with community inclusion

    Assistance with housekeeping

    Assistance with medication management and medical services

    Assures compliance with agency policies and procedures, contractual obligations, and state and federal regulations

    Assures and supports client choice

    Works to increase client independence

    Maintains a cooperative relationship with families and guardians of clients, as well as with team-members within your department and other agency departments, and with all collaborating community agencies and service providers.

    Plan, adapt and implement activities, which enhance the personal growth, community inclusion and vocational skills of each participant and ensure that the participant is an active partner in determining their plan and activities.

    Using discovery and other assessment techniques, assess individuals interests, skills and support needs and develop plan and activities based on the information obtained.

    Schedule, facilitate and oversee activities that promote independence and employment preparation such as community mapping, tours, volunteering, travel training, independent skill acquisition, socialization and community involvement.

    Operate and utilize agency software needed to carry out job responsibilities while maintaining security and ensuring confidentiality.

    Responsible to assist in making staff know and understand the responsibilities with the EVV system

    Develop and structure curriculum based on the acquisition of daily living skills, participant preferences, goals and fostering independence.

    Complete documentation detailing skills, abilities, areas for additional training and growth as well as provide input to assist in the implementation and documentation of each participants service plans, assessments and objectives.

    Promote and enforce safety as well as model proper social conduct in all settings in which services are being rendered : homes, businesses, places of employment and the general community.

    Gather, organize, requisition and store supplies and materials as needed

    Skilled in communicating in spoken conversations, skills in writing and reading.

    Provide and assist with transportation including mobility training, coordination and scheduling of transportation services.

    Attend job related meetings and trainings

    Identifies strengths and gifts of clients and provides support that emphasizes and promotes the further development of individual strengths and gifts.

    Incorporates informed choice in discussions of risk

    Explores and fosters natural relationships

    Administers prescribed medications under written direction of Physician or as directed by Management.

    Maintain client rights and confidentiality

    Assures all charting is up-to-date and signed off on in client binders.

    Represent Opportunity Inc. in a professional manner at all times.

    Demonstrate excellent communication skills, accountability and responsibility.

    Effectively problem-solve and make decisions that align with the agencys Mission, Vision, Values and Goals.

    Qualifications :

    To perform this job successfully, an individual must be caring and compassionate, and want to make a difference in the lives individuals with disabilities. Capable of being firm but have sensitivity and able e to use tact and diplomacy in dealing with fellow employees, workers, families, guardians, general public and other stakeholders. Able to work with limited daily supervision and make accurate, rapid, independent decisions. Good communication skills. Exceptional interpersonal skills.     Education / Experience :

    High school diploma or general education degree (GED); or one to three months related experience and / or training; or equivalent combination of education and experience.

    Certificates and Licenses :

    PCW certification

    Transportation Department Drivers Training

    o Valid drivers license with a good driving record may be required based on the current needs of the agency at the time

    Opportunity Inc. will provide all necessary training

    Previous PCW certification, CNA license, or other required training certificates are transferrable
