Position Summary

The Care Coordinator is responsible for assisting patients with transitioning between various care facilities within nephrology, which include post-acute transition, transplantation, dialysis, and external specialists, including vascular surgery. This role requires strong attention to detail, excellent communication, and the ability to navigate payer and health system portals efficiently. A strong knowledge of the local health systems, dialysis providers, and other nephrology-related providers is necessary.

Key Responsibilities

  • Collaborate with other team members to oversee transitions of care.
  • Communicate with providers, patients and their caregivers to proactively set expectations of the processes involved in the applicable transition.
  • Communicate with patients and their caregivers to prepare and submit paperwork and complete required testing.
  • Refine and develop documented workflows for care transition.
  • Initiate and complete scheduling for procedures ordered by providers.
  • Initiate and manage prior authorizations for those procedures where the facility does not obtain their own.
  • Verify insurance coverage as applicable.
  • Monitor and track patients throughout the transition process , follow up with payers, and update patients and providers on status.
  • Document all steps and communications clearly in the EHR.
  • Monitor inpatient stays and discharges for timely follow up with a practice provider
  • Gather clinical documentation needed for transitions of care (hospital notes, diagnosis codes, treatment rationale, office notes, diagnostic records, etc.)
  • Maintain an organized log for vascular access, transplant, and dialysis starts, with submission dates, facility / provider, and access placements.
  • Coordinate provider signatures or additional documentation for appeals or peer-to-peer reviews.
  • Stay current with payer-specific policies (BCBSM, Medicare / Medicaid, Optum, etc.).
  • Assist our scheduling specialists with setting up infusions and biopsies.

Qualifications

  • Previous experience in medical clerical work required.
  • Strong knowledge of local health systems and communities.
  • Strong attention to detail and organizational skills.
  • Excellent communication skills, both verbal and written.
  • Teamwork attitude.
  • Great people skills and a friendly, professional demeanor
  • Ability to focus and manage multiple tasks in a fast-paced environment.
  • Able to handle a high volume of calls
  • Proficient with computers and data entry, Microsoft Office and EMR systems
