Job Title : Registered Nurse

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : Minimum of 2 years home health experience

License : Current state RN licensure

Certifications : Current BLS certification

Must-Have :

• Minimum of nine months of clinical nursing experience
• Ability to effectively communicate with patients, families, and hospital personnel
• Strong interpersonal and customer service skills required to interact effectively with individuals from various cultural and socio-economic backgrounds
• Must be able to lift, carry, transport, and move clients, if needed

Description :

Under general supervision of the nursing supervisor, utilizes the knowledge and application of nursing principles to provide direct patient care in the home.

Assesses the need for other allied health care services such as physical therapy, occupational therapy, nutrition, speech pathology, and social work, to ensure an integrated home health care delivery system.

Performs other related duties as required.

IV experience, OASIS experience, and Epic experience are preferred.

Mileage from the home site to patient homes will be tracked and reimbursed at a set rate.

Acute care experience is preferred.

Must possess a current driver's license and a satisfactory driving record.

Must have reliable transportation and a telephone.
