Role Overview
As a Hospice Registered Nurse , you will work under the supervision of the unit Supervisor / Manager / Director. You will be responsible for delivering patient care through the nursing process of assessment, planning, implementation, and evaluation. In this role, you will provide direction and guidance for patient teaching and the activities of ancillary personnel while maintaining professional nursing standards.
Key Responsibilities :
- Patient Assessment : Assess physical, psychological, and social dimensions and educational needs upon admission, completing a written assessment within the designated timeframe.
- Care Planning : Identify patient needs and formulate a written care plan within 24 hours of admission. Implement and document the care plan's effectiveness, revising and updating as necessary.
- Patient Support : Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Patient Independence : Promote patient independence by setting care goals and educating patients, friends, and families about the condition, medications, and self-care skills.
- Quality of Care : Adhere to therapeutic standards, measure health outcomes, and make necessary adjustments to care plans.
- Team Strategies : Utilize multidisciplinary team strategies to resolve patient problems and needs.
- Infection Control : Adhere to infection-control policies, medication administration, and controlled substance regulations.
- Continuity of Care : Document and communicate actions, irregularities, and continuing patient needs to maintain continuity of care among nursing teams and physicians.
- Leadership : Utilize leadership skills to delegate, communicate with, and evaluate team members.
- Crisis Management : Recognize changes in patient status and provide appropriate intervention and documentation.
- Patient Education : Provide ongoing education based on daily assessment of disease processes, care requirements, and expected recovery.
- Prioritization : Prioritize care based on individual patient needs.
