Shift : Monday - Friday 8am-5pm
Location : Fully Remote
Fully remote position. Will be calling patients that need orthopedic follow up or additional information about their injury for next steps. Provide information to the patient around their best option for care based on insurance, location , injury type and availability, RN license REQUIRED with Orthopedic experience d preferred

The Regional Orthopedic / Urology Navigator is responsible for the overall coordination and implementation of the hospital's Urology Service Line or Total Joint Replacement Program, Fracture Program, and Spine Program (as applicable per site). The Navigator will be guiding the orthopedic / urology patient and their family through AdventHealth's Orthopedic or Urology Program, from preoperative education through the continuum of care. This individual will have the ability to interact with patients and their families, physicians, their care partners, as well as several key hospital departments including surgery, rehabilitation and occupational therapy, nursing, case management, marketing, home health and others involved in the care of the patient. Regularly collects data through chart review and computer-generated reports. Presents data and reports to Orthopedic Subspecialty Governance Committee and other leadership groups as directed. Develops, facilitates, and monitors selected performance metrics and ensures compliance with all activities required to achieve and maintain a Disease-Specific Certifications with the certifying body. Works closely with orthopedic subspecialty surgeons to assist with data collection and community outreach. Regularly works in collaboration with the Regional Orthopedic Institute team.

Responsible for patient education included but not limited to 1 : 1 teaching, preoperative classes, developing and utilization of regional teaching materials and implementing and monitoring the family "coach" program.Develop and implement, in conjunction with marketing, a community outreach program for orthopedics / urology that includes but not limited to health fairs, physician lunch-ins, local community events, education events, speaking engagements.Utilizes and monitors a data tracking system for orthopedic / urologic outcomes through the care continuum such as patient satisfaction, surgical data, patient outcomes, and identifying areas that require improvement, in order to provide key statistics and outcomes at governance meetings.Manage, monitor and assess current quality metrics required by national organizations and evidence-based practice for optimal clinical outcomes.Facilitates Orthopedic Subspecialty Governance meetings with surgeons and multidisciplinary team members.Works with Regional Orthopedic Institute team on developing and reviewing orthopedic clinical policies and procedures.Works closely with marketing to develop patient education materials, community outreach material to help inform the community and increase market share.Develops, facilitates, and monitors selected performance metrics and ensures compliance with all activities required to achieve and maintain Centers of Excellence and / or Disease-Specific Certifications with the certifying body.May conducts daily rounds on in-patients with physicians to ensure desired patient outcomes. Coordinates care issues with the surgeon and other members of the interdisciplinary team.Works closely with Case Management concerning post-op discharge planning and decreasing length of stay.Works in partnership with nursing to develop strategies to improve HCAHPS scores to meet or exceed the organizations’ established goals.Works in conjunction with clinical educators and nursing staff to ensure competency and understanding the needs of the orthopedic patient.Acts as a resource to patient care staff of the Orthopedic inpatient unit.Ensures physician satisfaction by monitoring their needs, developing relationships and following through with problem resolution, recognition and performance improvement, including regular visits with surgeons' office staff if needed.Facilitates working with other campuses, regional team and finance to prepare and implement Bundled Payments according to Medicare guidelines.Strives for evidenced based practice and demonstrates critical thinking skills to ensure optimal, safe outcomesQualifications
The expertise and experience s you’ll need to succeed :
