Position : Inpatient Case Manager - RN - Days

This acute-care teaching hospital has been dedicated to providing outstanding medical care for over 135 years. Located in Park Slope, Brooklyn, we are seeking a Case Manager RN to oversee discharge planning and utilization management for a designated group of patients.

The Case Manager will play a vital role in assessing admission appropriateness and continue to evaluate patient care plans to ensure they are implemented effectively and promptly. Collaboration with physicians, social workers, clinical nurses, home care services, and other interdisciplinary team members is essential for maximizing the efficiency and quality of care provided.

Key responsibilities include :

• Managing 60-70 medical / surgical daily admissions, including risk identification and clinical reviews.
• Ensuring 100% clinical reviews for managed care, Medicare, and Medicaid admissions.
• Conducting concurrent and retrospective reviews.
• Communicating with medical staff regarding patients who do not meet inpatient criteria.
• Coordinating expedited appeals for inpatient denials with attending physicians and insurance providers.
• Maintaining efficient organization resource utilization.
• Completing patient assessments within 24 hours.
• Planning safe and effective post-acute care options.
• Securing necessary authorizations from managed care companies for post-discharge services.

This is a full-time position with hours from Monday to Friday, 8 : 30 am to 5 : 00 pm, and will require weekend rotations and availability for holidays.

Please note that all roles require on-site presence. Employees should live within a commutable distance to NYP, as travel expenses are not reimbursed.

Salary Range : $118,134 - $155,787 / Annual
