Job Title : Registered Nurse (RN) Case Manager - Home Health Specialist

Company : Home Health Care, Inc.

Location : Greater Twin Cities Metro Area

Schedule : Full-Time (Monday to Friday, 8 : 00 AM - 5 : 00 PM), 1 weekend / month, 1 on-call shift / week

Compensation : $85,000 - $130,000 annually + $6,000 in bonuses during first year of employment
