PACE North (Program of All-Inclusive Care for the Elderly) helps adults 55+ who qualify for nursing facility care remain independent and safe in their homes while receiving the support they need. Our team provides care both at our state-of-the-art day center on Garfield near Hammond, and in participants’ homes throughout the community.
