Role Overview :

  • Collaborate with interdisciplinary teams to organize and deliver evidence-based substance use treatment services while upholding ethical professional standards.
  • Conduct comprehensive bio-psychosocial assessments and develop tailored treatment and discharge plans, collaborating closely with the team to address patient and family needs.
  • Educate patients and families on treatment goals, covering disease prevention, advocacy, and adherence to treatment regimens.
  • Provide direct patient care, which may include case management, crisis intervention, counseling, and emotional support during challenging times.
  • Maintain timely and thorough documentation reflecting patient assessments, interventions, and outcomes per regulatory and third-party standards.
  • Participate in administrative activities, including team meetings and clinical supervision.
  • Contribute as a team member to enhance the overall patient experience.

Qualifications :

  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.
  • Experience in a healthcare setting focusing on substance use disorders or addiction services.
  • Hold an active LCSW, LADC, or LPC license.
  • Valid Driver's License.
  • Computer proficiency.
  • Able to assess problems, make decisions, and implement solutions promptly.
  • Utilize supervision and teamwork effectively in patient assessment and interventions.
  • Strong communication skills essential for building relationships across the organization.
