Home Health Nurse Practitioner

Position Details :

• Certifications : BLS required
• Experience : Minimum 1-2 years in acute care, hospital, or home health / hospice settings
• Start Date : Within 2-3 weeks after offer (quick feedback response time)
• Schedule : Full-time, Four 10 hour shifts, rotating between Mon - Fri, 8 : 00 AM - 7 : 00 PM (40 hours / week)
• Duties : Travel to patients' homes to provide care, conduct assessments, and complete documentation
• Patient Volume : 3-5 patients / day
