Job Summary
- The Registered Nurse - Long Term Care (RN-LTC) provides comprehensive nursing care in a long-term residential setting. This role ensures resident safety, comfort, and dignity while managing their complex medical, physical, emotional, and psychosocial needs. The RN-LTC functions in accordance with established policies, regulatory standards, and nursing best practices, promoting a high level of resident satisfaction and quality of life.
Essential Functions
Performs nursing assessments and reassessments based on resident needsProvides direct care and administers medications and treatments per protocolsMaintains timely and accurate documentation of resident care and conditionsCommunicates effectively with residents, families, physicians, and interdisciplinary team membersDevelops, updates, and evaluates individualized plans of care in collaboration with residents and familiesAssesses and manages resident pain levels; educates residents / families on pain management strategiesApplies infection control practices and maintains a safe, clean environmentAssists residents with activities of daily living, including hygiene, mobility, nutrition, and eliminationProvides emotional support and advocates for resident rights and dignityParticipates in resident admissions, discharges, and transfersReports significant changes in resident conditions and follows up as appropriateFunctions as a liaison between residents, families, healthcare providers, and facility leadershipParticipates in staff orientation, training, and performance improvement activitiesKnowledge / Skills / Abilities / Expectations

Education
Associate Degree in Nursing (ADN), requiredBachelor of Science in Nursing (BSN), preferredLicenses / Certifications
Current state RN license or current Compact State RN licenseBasic Life Support (BLS) Certification (AHA) required within time frame specified in facility policyExperience
Previous long-term care or geriatric nursing experience preferred
