Job Description

The position is for Licensed Clinical Social Workers (LCSWs) in Connecticut, focusing on psychotherapy services in skilled nursing and assisted living facilities. The role involves diagnostic evaluations, individual / group therapy, collaboration with multi-disciplinary teams, and maintaining professional relationships with staff.

Responsibilities :

• Conduct diagnostic evaluations and provide individual / group therapy for residents in skilled nursing and assisted living facilities.
• Collaborate with multi-disciplinary teams at each facility, offering feedback and assistance with behavioral management techniques as needed.
• Develop and maintain professional relationships with staff, educating them on appropriate referrals to maintain and build caseloads.
• Work with other members of the interdisciplinary team assigned in each building.
• Participate in case reviews and utilization reviews as needed.
• Maintain timely and accurate clinical documentation for patients on caseload.
