As a Clinical Case Manager, you will provide a broad range of services within the guidelines established by the Organization and specific programs. You will work with a high degree of independent accountability, adhering to professional and ethical standards, organizational policies, and contractual requirements. The populations you will serve may include children at risk of human trafficking, adults who have experienced trafficking, families, immigrants, refugees, and individuals with mental illness, HIV/AIDS, and/or substance abuse issues. Your responsibilities will include connecting participants to healthcare, behavioral health, and social services such as housing, adult education, and employment assistance programs. Your individual accountabilities and work volume will be established through the development of annual Success Objectives, providing a clear framework for your professional growth and impact.

Essential Functions:

Duties & Responsibilities

  • Provides clinical case management services to program participants through a short-term case management model. Provides intense advocacy, crisis intervention, and makes appropriate referrals to other Organization units and external providers as needed. Collects informed consent for program participation and the non-clinical assessments.
  • Meets regularly with assigned cases and representatives from community as needed. Develops, maintains, and follows up with program participant treatment plans to ensure that realistic goals and deadlines of accomplishments are being met.
  • Conducts outreach and intake as requested. Receives case assignments, create individualized care plans, tracks community linkages, reviews case progress and determines case closure through the supervisor of the assigned program and in accord with funder requirements.
  • Develops service plans, which outline a specific plan of action with each program participant under the supervision of the supervisor of the assigned program. Screens for income supports for benefits and entitlements to advocate on the behalf of program participants.
  • Accurately completes in a timely manner all necessary forms, case recordings and statistical reports.
  • Develops, monitors and enhances professional relationships with social service, health and governmental agencies. Establishes and maintains a professional relationship within the guidelines of professional standards, program participant profiles, confidentiality laws and the newest clinical research available.
  • Participates and contributes in regular supervision meetings, team meetings, in house training sessions and conferences. Participates in program and operational planning.
  • Maintains the volume of work designated by project standards and utilizes strengths-based theory and concepts integration in clinical practice. On an ongoing basis assimilates the knowledge of resources available to implement clinical concepts.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies and Unit guidelines. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Other duties may be assigned.

Education and Experience:

  • BA/BSW and three (3) years’ experience or Master’s degree in social work, counseling, clinical psychology or related field
  • Bilingual strongly preferred.

Certifications/Licenses:

  • Based on the primary duties of this position, an LPC/LSW license may be required. Certified Domestic Violence Professional preferred.
  • A valid driver’s license may be required.

Knowledge/Skills/Abilities (K/S/A):

  • Preferred ability to speak and fluency in a second language including but not limited to the following multi-language abilities:
    • Ability to read and interpret documents such operating and maintenance instructions, and procedure manuals.
    • Ability to write routine reports and correspondence.
    • Ability to speak effectively before groups of customers or employees of organization.
  • Access to regular, dependable, and timely transportation to main work site and to scattered sites.
  • Ability and comfortability working with people of varying socioeconomic backgrounds.
  • Ability and comfortability working providing trauma-informed care and patient centered approach.
  • Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio, and percent.
  • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, Salesforce; Windows operating systems; and other software routinely used by Heartland Human Care Services.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.

Work Arrangement:


The role is a “hybrid” or “remote” arrangement, primarily involving home-based work and community engagement to meet participants, it is expected this person would be able to meet for “in person” meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.

Work Environment:


  • The noise level in the work environment is usually moderate.
  • May be required to work evening and/or weekend hours as needed.
  • May require travel throughout the Chicago area to participant homes and community locations.
  • Program sites are located throughout Chicago, including the North, West, and South sides of Chicago. Employees may be required to report to any of our locations on a temporary or permanent basis.
  • For homework, the employee must have a confidential workspace.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • The employee is regularly required to sit, stand and walk.
  • The employee must be able to stoop, kneel and/or crawl.
  • The employee is regularly required to use hands to key, handle, or feel and reach with hands and arms.
  • The employee must occasionally lift and/or move up to 25 pounds.
  • The employee must have unrestricted ability to provide physical restraint.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
