CLASS TITLE : Community Support Specialist
Under the general supervision of the Aging Director of the Buffalo Trace ADD, is responsible for providing citizens with a trusted place for gaining information and assistance related to services and supports for individuals of all ages but especially those who are aged or those with disabilities and their caregivers.
ESSENTIAL JOB FUNCTIONS :
- Provide information and assistance to both public and private-pay individuals about services and supports for which they are eligible.
- Involve stakeholders, consumers, state agencies, providers, and public and private partners in planning, implementing, and evaluating the ADRC.
- Assist the ADRC Coordinator in marketing the Aging & Disability Resource Center as a single point of entry to publicly funded programs.
- Provide access, eligibility screening, assessment of long-term care support needs and care planning to callers on the Aging and Disability Resource Center Phone line.
- Assist with Level I Screenings for service eligibility and maintenance of the waiting list.
- Document all calls in the state data system for client tracking and the development and maintenance of the resource database.
- Provide assessment and case management services for functionally impaired older persons and disabled individuals in the Buffalo Trace Region under the Homecare and Title III Older Americans Act Programs.
- Determine eligibility for services, develop a plan of care, arrange for service provision and ongoing monitoring of client cases.
- Communicate appropriate assessment information to appropriate provider agencies.
- Assure that service providers are accountable for services provided, that services are activated by individual need and that duplication or over-utilization of services is avoided.
- Responsible for identifying and documenting gaps in service and mobilizing resources to fill these gaps.
- Provide outreach and target services to those with disabilities and their caregivers as funding is made available.
- Assist in conducting public forum, consumer satisfaction surveys, needs assessments, etc., which can be used in the ongoing design and evaluation of the program.
- Assist in coordinating program activities with the Area Agency on Aging and Independent Living services.

Degree Required : Bachelor’s or master’s degree in a Human Services field such as social work, counseling, sociology, or psychology; or possess a bachelor’s degree in a field not related to human services with two years of experience working with the elderly or disabled or special needs population;Skills Required : Must have working knowledge of computer systems and various software; must possess good organizational skills; ability to complete forms and analyze data; ability to work well with others; good verbal and written communication skills.Must possess a valid drivers license.
