Job Title : RN Home Health.

Job Requirements :

Conduct comprehensive patient assessments, including OASIS documentation, to develop individualized care plans.

Deliver skilled nursing care in accordance with physician orders and agency policies.

Administer medications, wound care, and other prescribed treatments.

Collaborate with interdisciplinary team members (PT, OT, MSW, HHA) to ensure coordinated patient care.

Educate patients and caregivers on disease processes, medications, and self-care techniques.

Monitor patient progress and update care plans as needed to reflect changes in condition.

Accurately document all visits and communication using HCHB in a timely manner.

Participate in case conferences and maintain ongoing communication with the care team.

Travel within assigned territories to provide direct patient care.

Required Documents : BLS AHA

RN WI License
